Improving cardiovascular health in cities requires more than a strong public health ambition. It needs a more practical way to understand local needs, set priorities, align partners, choose interventions and track whether they are working.
We have developed comprehensive CARDIO4Cities implementation tools to help city teams move from evidence and local data to coordinated action. The Playbook provides a step-by-step process for designing and implementing a cardiovascular population health roadmap. CARDIO4Cities in Practice complements the Playbook with concrete intervention options drawn from partner cities and linked to the CARDIO pillars.
Together, these two tools can help cities translate the CARDIO4Cities approach into locally owned action that can be tested, refined and scaled.
CARDIO4Cities Playbook

The CARDIO4Cities Playbook outlines our evidence-based approach to improving cardiovascular population health allowing new cities to take action. The playbook provides a process to set ambitious yet realistic targets, developing a population health roadmap with concrete interventions, responsibilities and timelines, and tracking progress to adjust interventions as needed. The Playbook is the starting point for implementation.
CARDIO4Cities in Practice

In addition to framing this approach, the CARDIO4Cities Playbook incorporates CARDIO4Cities in Practice. CARDIO4Cities in Practice provides concrete intervention options for each pillar, validated in partner cities. These interventions have been implemented in CARDIO4Cities cities and have been validated to positively impact control rates of at least one of the three main cardiovascular (CV) risk factors (hypertension, hypercholesterolemia and diabetes).
